Palliative Care

Organization:  High Point Regional Health System, High Point, NC.  The system鈥檚 innovative highlights included integration of patient-centered care and palliative care throughout, strong team model for patient care, and leadership by chaplains.
Organization: Continuum Hospice Care, New York, NY.  Innovations highlights were the hospice鈥檚 Open Access focus on clinical indicators rather than prognosis, reliance on evidence-based medicine, and elimination of cost considerations and complexity as factors in hospice eligibility.
Organization:  Covenant Hospice, Pensacola, FL.  The hospice鈥檚 innovations include its culture of leadership throughout the organization, strong educational outreach, and evidence-based tools and protocols.
Organization:  Pain and Palliative Care Program, Children鈥檚 Hospitals and Clinics of Minnesota, Minneapolis/St. Paul, MN.  Innovation highlights noted were a three-part focus on pain management/palliative care, end-of-life care, and integrative medicine; emphasis on 鈥渇ollow the kid;鈥 and鈥
Organization:  Four Seasons, Flat Rock, NC. Innovations cited were leading-edge palliative care delivery in rural setting, collaborative research with university, and palliative care outreach. Recent Achievements:
Organization:  The Department of Veterans Affairs VA New York/New Jersey Healthcare Network, Brooklyn, NY. Innovation highlights were a strong staff education and training program, expansion of palliative care services in intensive care units, community outreach, and data collection to drive鈥
Organization: The Center for Hospice & Palliative Care, Cheektowaga, NY. Innovation highlights were its broad range of service, strategic approach to understanding and meeting community needs and collaboration across the health care continuum.
Organizations and groups that provide palliative or end-of-life care may apply through Aug. 13 for the 2019 Circle of Life Awards, which honor outstanding and innovative palliative and end-of-life care programs.
By helping our communities better understand advance directives and health care decision-making at the end of life, we can demystify the topic and increase the number of people who have a plan in place.
