Tragic incidents that result from medical errors should not be <a href="https://nap.nationalacademies.org/catalog/9728/to-err-is-human-building-a-safer-health-system" target="_blank">criminalized</a>. When errors happen hospitals and health systems need open lines of communication to identify and understand the series of events so they can update patient safety systems to further prevent errors.
