Maureen Bisognano, a senior fellow at the Institute for Healthcare Improvement (IHI), made this simple yet profound statement 12 years ago at the IHI National Forum on Quality Improvement in Health Care.</p><p>Her keynote address posed a transformational challenge to the field as leaders struggled to meet rising market demands for an excellent patient experience and to make a meaningful impact on the patients they serve.</p><p>Then, as now, many health care executives struggled with how to design systems of care and processes with continuous improvement built into operations. And when these efforts didn’t deliver intended results, leaders and culture were often cited as the reasons.</p><h2><span>Systems Need Fixing More Than People</span></h2><p>Ensuing modifications to improvement plans usually called on leaders to set bolder goals, enable staff to do their best work and respond to employee feedback. These were not bad ideas, but they often were flawed, say three IHI leaders in a recent <a href="https://hbr.org/2024/07/to-improve-health-care-focus-on-fixing-systems-not-people" target="_blank" title="Harvard Business Review: To Improve Health Care, Focus on Fixing Systems — Not People">Harvard Business Review report</a>.</p><p>The authors argue that care delivery systems need fixing far more than the people who work in these systems. Going forward, health care leaders must account for the strains on the workforce of poorly designed and unoptimized systems and create systems-focused improvement programs that everyone owns together.</p><p>Organizations like Jefferson Health in Pennsylvania, Prisma Health in South Carolina and the Greater Baltimore Medical Center have built dynamic operating models that support continuous learning and improvement via system redesign, the report notes. Many others have built aspects of these programs, which the authors call successful, to be operationally integrated, tech-enabled care operating systems.</p><p>So, what does it take to build dynamic operating systems to support continuous improvement?</p><h2><span>4 Traits of Systems-Focused Improvement Organizations</span></h2><h3><span>1</span> <span>|</span> They recognize and celebrate staff commitment and passion.</h3><p>Harness the brilliance of your team on a massive scale and celebrate them for their resilience and commitment to excellent care.</p><h4><span>Takeaway</span></h4><p>Look for opportunities to recognize individuals and teams for perpetuating continuous improvement. For example, Jefferson Health leaders during quarterly enterprisewide meetings celebrate individuals and teams that “make a great catch,” such as identifying a potential safety risk that leads to large-scale improvements and risk mitigation, the report states.</p><h3><span>2</span> <span>|</span> They obsess over system improvements.</h3><p>Daily workflows are the heart of these operating systems. Quality and safety team leaders concentrate day in and day out on methods to obtain feedback about how operating systems are performing.</p><h4><span>Takeaway</span></h4><p>This leads to daily identification, triage and resolution of real-time operational challenges. “In these organizations, the quality team is not seen as reactive, policing or scorekeeping; it is viewed as front-line clinicians’ greatest advocates,” the authors wrote.</p><h3><span>3</span> <span>|</span> They go beyond rigorous process-improvement methodologies.</h3><p>Programs like Lean Six Sigma, Total Quality Management and others aimed at creating high-reliability organizations have guided health care toward production processes proven in other fields. And yet, many health care organizations have achieved only incremental improvements using these efforts. One key reason may be that they haven’t successfully engaged workers and customers to drive daily learning and continually optimize system performance.</p><h4><span>Takeaway</span></h4><p>The key is for those leading efforts to improve organizational improvement to focus less on trying to get more from their workforce and more on trying to maximize what they can get from the system. This is a critical concept when considering health care’s challenge of overwhelming burnout and global shortages of physicians and nurses, the authors state.</p><h3><span>4</span> <span>|</span> They thrive on feedback.</h3><p>Successful systems-focused industries don’t regularly spend time with front-line workers discussing abstract methods, principles or theories for how they can improve their work. They devote time to seeking feedback and understanding challenges so they can design or redesign daily operations to make front-line workers’ jobs easier while simultaneously improving system output.</p><h4><span>Takeaway</span></h4><p>Quality and safety cannot be ancillary concerns. They must be embedded in the work itself. (HATCo)</a> to be led by former Intermountain Health CEO Marc Harrison, M.D.</p> <p>Harrison and General Catalyst CEO Hemant Teneja wrote a <a href="https://www.generalcatalyst.com/perspectives/the-future-of-health" target="_blank" title="General Catalyst: The Future of Health">blog</a> about their self-described plan to create an “Amazon ecosystem of health care” to help health systems achieve a more affordable, accessible and proactive care system.</p> <p>As part of its goals, General Catalyst will look to acquire a health system within HATCo’s first year as a national blueprint to transform the field and to put the system on a platform where digital systems can scale across the enterprise without operating in silos.</p> <p>HATCo also will work with GC’s 20-plus health system partners to help them on their transformation journey and will help to catalyze the health assurance ecosystem by building an interoperability model with tech-driven solutions from a subset of health care portfolio companies.</p> <p>HATCo will operate as a for-profit, Harrison said, adding that, “We are unaware of any industry that is able to truly transform without the impetus of capitalism behind it.”</p> <p>In their blog, Teneja and Harrison emphasize their focus on stakeholder alignment and that transforming health systems will require a “decades-long time horizon.” In its work, GC will emphasize this latter point to investors.</p> <h2><span>3 Takeaways from the Launch of HATCo</span></h2> <h3><span>1</span> <span>|</span> The company will focus on platform innovation.</h3> <p>It sees the current private equity model in health care as having a maniacal focus on removing costs from the system. Much of the companies’ focus has been on advancing their primary care reach and in-home care services while bulking up their physician networks.</p> <p>The recent news that <a href="https://www.wsj.com/articles/cvs-plans-to-bid-for-signify-health-sources-say-11659890029" target="_blank" title="The Wall Street Journal: CVS Plans to Bid for Signify Health">CVS Health plans to bid for Signify Health</a>, a value-based care analytics company that helps payers, employers and providers with in-home care, is the latest case in point. If successful, the deal would help CVS Health fulfill its growth strategy as a provider.</p> <p>Nevertheless, there’s a long way to go given that multiple suitors have their eye on <a href="https://www.signifyhealth.com/" target="_blank" title="Signify Health homepage">Signify Health</a>. But even if CVS Health doesn’t end up with Signify Health, the company made clear in a recent earnings call that it will pursue deals that meet its strategic goals in primary care and home health.</p> <h2>4 Ways a Signify Health Acquisition Could Help CVS</h2> <h3>1 <span>|</span> <span>Deliver a more robust provider network.</span></h3> <p>After acquiring the accountable care organization <a href="https://caravanhealth.com/resources/blogs-news/signify-health-completes-acquisition-caravan-health/" target="_blank" title="Caravan Health: Signify Health Completes Acquisition of Caravan Health.">Caravan Health</a> in February, Signify Health’s network now includes 200-plus health systems, 100 Federally Qualified Health Centers and more than 10,000 primary care practices, according to an <a href="https://www.insiderintelligence.com/content/cvs-health-eyes-signify-health-acquisition-here-s-what-means-retail-primary-care" target="_blank" title="Insider Intelligence: CVS Health eyes Signify Health acquisition—here’s what it means for retail primary care">Insider Intelligence report</a>. With the Caravan Health acquisition, Signify Health says it now supports about $10 billion in medical spending.</p> <h3>2 <span>|</span> <span>Provide a centerpiece for CVS’ primary care strategy.</span></h3> <p>Unlike Walmart, which primarily hires nurses to staff its retail clinics, CVS wants its new primary care clinics to be mainly physician-led. CVS Health’s primary care strategy is similar to that of Walgreens, which bought retail clinic partner <a href="https://www.villagemd.com/" target="_blank" title="VillageMD homepage">VillageMD</a> for $5.2 billion to locate clinics adjacent to its retail pharmacy stores.</p> <h3>3 <span>|</span> <span>Expand in-home health services.</span></h3> <p>Signify Health provides more than 1.9 million in-home health assessments per year and connects with patients via short message services, email and telephone reminders between visits. In July, the company said it would be leaving the Centers for Medicare & Medicaid Services’ <a href="https://innovation.cms.gov/innovation-models/bundled-payments" target="_blank" title="CMS.gov: Bundled Payments for Care Improvement (BPCI) Initiative: General Information">Bundled Payments for Care Improvement Advanced Model</a> to focus on its home and community services segment — aligning with CVS Health’s aims.</p> <h3>4 <span>|</span> <span>Connect with underserved consumers.</span></h3> <p>With roughly 9,900 stores throughout the U.S. and Puerto Rico, including those located in underserved areas, CVS has a significant opportunity to provide primary care in those communities.</p> <h2>Convenient Access to Care Will Be King</h2> <p>The fact that the CVS Health news comes just weeks after <a href="https://press.aboutamazon.com/news-releases/news-release-details/amazon-and-one-medical-sign-agreement-amazon-acquire-one-medical" target="_blank" title="Amazon Press Release: Amazon and One Medical Sign an Agreement for Amazon to Acquire One Medical">Amazon said it will buy the primary care organization One Medical</a> for nearly $4 billion is significant. Competition is heating up among new providers to transform delivery, with consumer-centric, tech-supported efforts to make every step in the patient experience more convenient.</p> <p>Some see Amazon’s One Medical purchase as a precursor to an eventual health care service subscription offering. “Whether Amazon directly adds health care to Prime or has an additional fee, or whether they work with employers to somehow weave in benefits for Prime subscribers, it seems inevitable that Amazon will leverage the subscription model,” notes Dan Clarin, managing director of Kaufman Hall’s strategic and financial planning practice, in a recent <a href="https://www.kaufmanhall.com/insights/article/amazon-and-one-medical-whats-going-here" target="_blank" title="Kaufman Hall: Amazon and One Medical: What’s Going on Here?">roundtable discussion</a> among the consulting firm’s leaders.</p> <p>CVS, meanwhile, is working with telehealth company <a href="https://www.fiercehealthcare.com/health-tech/amwell-scores-cvs-tie-roll-out-retailers-virtual-primary-care-service" target="_blank" title="Fierce Healthcare: Teladoc rival Amwell scores CVS tie-up to launch retailer's virtual primary care service.">Amwell to roll out the retailer’s new virtual primary care service</a>. The platform will give consumers and eligible Aetna and CVS Caremark members access to primary care, on-demand care, chronic-condition management and mental health services.</p> <p>Finally, CVS and Amazon see their roles as more than disrupting or transforming care delivery. They are out to change traditional health insurance — Amazon through its <a href="https://amazon.care/" target="_blank" title="Amazon Care homepage">Amazon Care program</a> and CVS through its <a href="https://www.aetnacvshealth.com/" target="_blank" title="aetna CVS Health: Introducing Aetna CVS Health™ ACA plans.">Aetna subsidiary</a>. What might the landscape look like going forward?</p> <p>The business model for Optum, VillageMD, CVS, ChenMed and now Amazon Care and One Medical is a network-restriction play, the Kaufman Hall team notes. Matthew Bates, managing director of enterprise service line lead, adds, “The entire business model of each of these companies is to route patients to the optimal place to get care and get as many expensive ancillary services as possible out of the mix. Providers must establish a comprehensive, documented data strategy that outlines the organization’s approach to data governance, top priorities for data usage and a road map to achieve greater data liquidity. Data liquidity, or the ability for data from all sources in an organization to easily flow through all systems, enables hyperpersonalized digital experiences. This is how online retailers know exactly what we’re looking to buy before we do. For health care organizations to anticipate patients’ needs in a similar fashion, hospitals and health systems must first create an underlying data foundation.</p> <h3>3. While the concept of personalization tends to invoke thoughts of cutting-edge medicine or splashy, concierge-like digital experiences aimed at the well-insured, personalizing care experiences may be most important for patients who have historically experienced health inequities. Providers’ digital transformation efforts must remove friction for all patients. As a starting point, providers should collect data on patient racial, ethnic, language, sexual orientation, gender identity and social factors that can be fed into various patient-facing and clinician point-of-care interfaces to drive more personalized care.</p> <h3>4. While they may not be able to solve all challenges created by societal impacts facing a patient, provider organizations can serve as an anchor among a network of community service providers, connecting patients to needed services and providing a warm, direct handoff to a person when possible.</p> <p>That same strategy increasingly is being deployed toward community investment. Hospitals and health systems invest a percentage of their earnings to ensure long-term financial stability for the organization. Typically, these investments are in the stock market or other traditional investment vehicles. And, some hospitals earmark a portion of their investments for community-based entities. For example, 1% of Cleveland Clinic’s reserves are invested in affordable housing, opening grocery stores in food deserts or other community revitalization projects.</p> <p>Boston Medical Center Health System (BMCHS) makes investments based on such criteria as how the funds can create jobs for people in the communities it serves.
