For their work with the AHA Suicide in the Health Care Workforce Collaborative, MediSys chose to focus on mitigating job-related stressors for their employees. Members from JHMC and FHMC worked together to create a plan that could benefit the entire network. By forming a team, members from MediSys were able to leverage their limited financial resources as well as staff bandwidth.</p><p>The MediSys team considered many interventions for their organization when starting out with the collaborative, but as a safety net institution, cost effectiveness was a key consideration. Because of its potential for widespread impact, they ultimately chose to implement a peer support training program at both of their hospitals.</p><p>Building a peer support program from the ground up was an ambitious goal to achieve within the time allotted for the collaborative, so the team at MediSys has adjusted their approach and is working toward implementing the program step by step. They have identified the peer support program model that they want to implement, and the program is both cost-effective and robust in the services and resources it provides to the workforce. The team who is leading this implementation has completed train-the-trainer courses so they can begin to roll out the training to others within the organization. The team has also recently completed Stress First Aid training. They employ over 38,000 individuals across 18 medical centers and hospitals throughout the state. For their work with the AHA Suicide Prevention in the Health Care Workforce Collaborative they chose to focus on addressing job-related stressors through peer-to-peer support programs.</p><p>IU Health decided to re-design their peer-to-peer support program as well as expand its offerings as part of their work with the collaborative. They initially attempted this program during the height of the COVID-19 pandemic, but it became lost in the shuffle due to the public health emergency’s challenges. In their initial launch, IU Health released a Psychological First Aid toolkit for leaders as well as a training for leaders to attend. While most of their leaders did attend this initial training, unfortunately, IU Health did not collect data on the programming and thus lost track of who had attended. IU Health also launched a team member peer-to-peer support program. Because they did not link these programs together or keep track of data the programs were found to be unsustainable – this was also impacted by changing organizational priorities in the wake of the COVID-19 pandemic.</p><p>Joining the collaborative at the start of 2023 provided IU Health the opportunity to put new eyes on the project. Their focus was sustainability of the program. Initially, they identified one hospital within their system that was in need of support. They have since added a second hospital to the pilot. Leaders within their hospitals are to identify members of their team/unit who they believe would succeed as a peer supporter. These individuals will complete peer-to-peer training that includes pre- and post-tests, virtual role playing, in-person sessions and self-study. The team at IU Health will collect the data and feedback from this pilot to determine any improvements that could be made before implementing it across the entire system. This training will, eventually, be required across their system. At this point they have received positive feedback from individuals who have already completed this training. Members of the team at IU Health note that in their role-play sessions they can see how intentional the trainees are in applying the concepts they have learned.</p><p>In addition to peer-to-peer support training, IU Health has developed an emergency app for mobile devices. Those who have completed the peer training, as well as those who have been supported, have access to the information on this app. IU Health ran into some technical difficulties with this app, causing some delay in its launch, but have since fixed the problems and are ready to launch the app to the supporters in the next steps of the program. They are one of the top three employers in Knox County and currently have approximately 1,400 employees. Through their work with the AHA Suicide Prevention in the Healthcare Workforce Collaborative they addressed job-related stressors. Knox Community Hospital chose three avenues in which to address these stressors, including debriefing sentinel events, providing employee education and increasing awareness of employee resources.</p><p>Following the height of the COVID-19 pandemic, much like many hospitals across the country, Knox reported seeing an increase in stress and burnout among staff. Employees were reporting feeling a lack of support that was exacerbated by chronic short staffing. The team at Knox wanted to provide their staff with the tools they need to handle difficult situations at work and foster a more positive attitude. One part of their project with the collaborative was focused on employee education. They were able to partner with the Knox County Mental Health and Recovery Board, who wrote many training courses for the hospital for free. The hope is that 60% of their employees complete at least one training course by the end of 2023. Some of the trainings they are offering include Mental Health First Aid; Question, Persuade, Refer (QPR); stress management; and Triple P – a positive parenting class. During their June Safety Fair, the team at Knox provided staff education on the 988 Suicide Prevention Hotline.</p><p>Another goal of their project was to increase awareness of employee resources that are already available to their workforce. The team focused on advertising and sharing the resources integrated in their EAP, including a free crisis coaching call line. Through employee surveys, they learned that much of their workforce was unaware of the programs already offered. Leveraging and increasing awareness of these programs was a very cost-effective way to increase employee support and access.</p><p>One obstacle that Knox Community Hospital experienced was low general interest in the activities they were offering to staff. They wanted to generate interest and subsequently increase staff participation in mental health activities across the board. One way that they addressed this barrier was by using rewards. They began offering incentives like continuing education credits and snacks to help facilitate interest. These rewards may help bring employees in, but they hoped that after participating interest in the programs would grow organically. Knox has also heavily been pushing their training to leadership within their organization. As leaders complete training, they will be able to bring knowledge to their team and encourage team members to attend the training as well.</p><p>In the coming months, Knox Community Hospital will continue to increase awareness around their available well-being resources, provide opportunities for training and share feedback with staff on their efforts to encourage continued participation in their initiatives. For their work with the AHA Suicide in the Health Care Workforce Collaborative, Bulter chose to address job-related stressors.</p><p>Butler Hospital is an inpatient psychiatric hospital and initially struggled to identify an employee training program suitable for this setting. Because of its setting, many of Butler’s staff already have unique skills in coping with potential conflict and violence. As leaders of the team at Butler attended the collaborative meetings, they began to feel a peer support program would be the most effective intervention for their organization. The team at Butler concluded that they were going to abandon their original ideas and design and implement something new. Leaders at Butler hospital felt their staff were most likely to talk with each other since, due to many barriers, many do not utilize local, outside resources. A major factor impacting this is due to the small size of Rhode Island, with an equally small psychiatric community. Lack of perceived privacy can amplify the stigma associated with accessing behavioral health resources.</p><p>After considering many options, the Butler team created a plan. They would start by offering to the extended executive leadership team an educational program, <a href="https://afsp.org/talk-saves-lives/" target="_blank" title="American Foundation for Suicide | About Talk Saves Lives">Talk Saves Lives</a>, that has a focus for health care workers. This program will eventually reach all operating units in their hospital as well as corporate leaders. The team at Butler identified that without the collaborative they may not have taken such a top-down approach. Through conversations and presentations within the collaborative Butler realized how important it was for there to be leadership support for these programs.</p><p>One of the units of their hospital has had a CISM (Critical Incident Stress Management) team in place for years, which the team found to be well-utilized by staff. A CISM is a peer-driven crisis response team that is designed to help individuals and teams recover more quickly from abnormally stressful job-related incidents and trauma. The second part of their plan is to ensure there are functional CISM teams in all operating units of the hospital. This will require an infusion of resources to provide the training and bulk up the program.</p><p>In the coming months and years, Butler Hospital will identify staff members to educate in a modified Question, Persuade, Refer (QPR) training. They are currently working on updating the referral resources associated with this program. Bryan Medical Center is part of Bryan Health, whose care reaches patients in parts of Kansas, Iowa and South Dakota in its regional focus. For their work with the AHA Suicide Prevention in the Health Care Workforce Collaborative, Bryan Medical Center chose to address job-related stressors.</p><p>The team at Bryan Medical Center administered a pre-survey to leaders throughout the organization in May 2023 to gauge the need for, and interest in, Question, Persuade, Refer (QPR) training prior to implementation. The results of the survey indicated a need for training and educating staff on behavioral health in order to improve skill and capability to respond to mental health crises. It also indicated the need for increased knowledge and confidence in accessing the mental health and well-being resources available to the workforce. The survey found that most leaders within their organization have not completed QPR training; furthermore, only 20% reported being familiar with Bryan Health’s policy on suicide prevention and less than half were aware of the National Suicide Prevention Lifeline. In the surveys, leaders at Bryan Medical Center had expressed that they lacked comfortability and skill when it came to responding to employees who were experiencing mental health concerns. Additionally, they identified a lack of knowledge throughout the organization regarding the policies and procedures in place for employee suicide prevention.</p><p>Bryan Medical Center identified eight employees who would become QPR-certified trainers. QPR training courses are designed to train “Gatekeepers” whose role is to recognize the warning signs of suicide and know how to offer help. The eight employees they chose represent different key departments throughout the organization including Chaplain Services, Rural Division, Education, Human Resources, Behavioral Health and College of Health Sciences. The departments identified were the ones most likely to respond or intervene when there is an employee or student in a crisis situation at this organization.</p><p>The team at Bryan Medical Center partnered with their director of behavioral health and learned that their intervention was aligned with the State of Nebraska’s Suicide Prevention plan. This realization enabled Bryan Medical Center to secure grant funding to cover their eight employees’ QPR training, as well as QPR booklets for future training. Bryan’s eight–employee team has since successfully completed their training.</p><p>Starting in September 2023, Bryan Medical Center has held monthly QPR training sessions for their workforce. Additionally, they will be providing all Lincoln, Neb.-based leaders with QPR training at an upcoming leadership meeting. Through their work with the AHA Suicide Prevention in the Health Care Workforce Collaborative they chose to focus on addressing behavioral health stigma throughout their system.</p><p>The team at Hartford HealthCare is working to remove or modify the questions from the hospital and system-based credentialing, peer review, and application policies and processes that stigmatize seeking care for a behavioral health disorder. Prior to joining the collaborative, Hartford HealthCare had already begun working on this project. One of the reasons that Hartford chose to go this direction was because they would be able to work on it with limited action from staff and leadership and there is little cost associated. Additionally, they felt this project could be accomplished in a discrete period and it would be easy to measure their progress.</p><p>The goal for Hartford was to remove language referencing mental health or substance use concerns in a way that is not supportive of or conducive of encouraging treatment-seeking behaviors. They also plan to review their process of requesting information about past mental health concerns or substance use that may not be relevant to their workforces’ ability to provide care at the present time. Through their participation in the collaborative, Hartford HealthCare had the opportunity to submit their credentialing questions for review and certification from the <a href="https://drlornabreen.org/" target="_blank" title="Dr. Lorna Breen Heroes’ Foundation | Fighting For The Professional Well-Being And Mental Health Of Health Workers">Dr. Lorna Breen Heroes’ Foundation</a>. Their work with the AHA Suicide Prevention in the Health Care Workforce Collaborative focused on combatting behavioral health stigma within their system. <p>Prior to their work with the collaborative, MedStar Health had already been prioritizing suicide prevention and employee well-being. They had implemented a few interventions that aligned with the AHA Suicide Prevention in the Health Care Workforce Guide and had identified an organizational goal of widespread Stress First Aid (SFA) training. They hoped joining the collaborative would help guide the expansion and implementation of this training. </p> <p>At the beginning of the collaborative, they created a timed roll-out around other organizational campaigns. They also worked to make the training material brief, with a focus on easy action steps and quick connection to resources. Initially, they planned to create a mandatory intro to SFA training for all associates. The organization had already completed smaller rollouts of this training that received positive feedback. One story the organization highlighted was following completion of SFA training, a nurse manager started to invite her staff to indicate their stress level using the Stress Continuum by placing the appropriate color circle next to their name on their assignment board. One day, one of her nurses places a red circle by her name, indicating she was in crisis. The nurse manager was able to pull her from patient care, meet with her and connect her with the appropriate mental health support resources. The success and impact this training had at a small level was a motivating factor in wanting to make it a mandatory program system wide. </p> <p>Being a part of an AHA led collaborative gave this organization’s team increased credibility leading to their initiatives becoming an increasing organizational priority. The team was also able to cite what other organizations were doing and, through the collaborative, had access to helpful materials and expertise. At this point, system wide, MedStar has trained 5% of their employees. For their work with the AHA Suicide Prevention in the Health Care Workforce Collaborative they partnered with the University of Kansas Medical Center, the organization’s teaching institution. For their work with the collaborative, University of Kansas chose to combat behavioral health stigma. </p> <p>At the start of this initiative, the University of Kansas team collected and reviewed data on staff engagement and utilization of behavioral health programs. Through this, they identified a lack of formal programming to train employees to act when witnessing harassment, discrimination, incivility, and lateral violence. They decided to implement bystander training in order to better equip their workforce in responding to these types of situations. Based on their data, the team identified specific populations and departments to receive this training first within their organization. These included residents, medical students, nursing students, OBGYN and mother/baby staff, emergency department staff, and environmental services. </p> <p>The University of Kansas system assembled an interdisciplinary team to develop the content for their standardized bystander training. They designed and piloted an in-person training as well as virtual training. The training program includes didactic learning, discussion, and case study review. Members of their workforce who completed the pilot training class were asked to fill out a pre- and post-course survey to determine the effectiveness of the course. The results of this survey have found that staff feel more comfortable intervening as bystanders after the training than they did before the training.</p> <p>In addition to piloting the bystander training among their target departments, the University of Kansas team is working to develop a toolkit for leaders.
