The AHA's 2016 Committee on Performance Improvement (CPI) engaged hospital leaders and experts in the field to identify key workforce challenges. These are challenges that already exist today and are expected to be further exacerbated as transformation continues and pressures mount with the various dynamics of a changing payment and delivery system. Transportation issues include lack of vehicle access, inadequate infrastructure, long distances and lengthy times to reach needed services, transportation costs and adverse policies that affect travel. Transportation challenges affect rural and urban communities.</p><h2><small>How is transportation related to health?</small></h2><p>Because transportation touches many aspects of a person’s life, adequate and reliable transportation services are fundamental to healthy communities. Transportation issues can affect a person’s access to health care services. These issues may result in missed or delayed health care appointments, increased health expenditures and overall poorer health outcomes. Transportation also can be a vehicle for wellness. Developing affordable and appropriate transportation options, walkable communities, bike lanes, bike-share programs and other healthy transit options can help boost health. This guide outlines transportation issues and the impact on health and health care access.</p><h2><small>What is the role of hospitals in addressing transportation issues?</small></h2><p>Multiple strategies may need to be employed to reduce transportation gaps for patients. This guide discusses the role of hospitals and health systems and recommends strategies for addressing transportation issues, including screening and evaluating patients’ transportation needs and providing transportation services through community partnerships or programs. They account for a disproportionate share of the nation's disease and health care spending.</p> <p>Hospitals and health systems are leading interventions to address the needs of high-need, high-cost patients. These hospitals and communities vary in location, service type, type of partners and degree of partnership. All participated in <em>Learning in Collaborative Communities</em>, led by the Health Research & Educational Trust and supported by the Robert Wood Foundation. Insights gained from site visits to the communities helped HRET create a companion guide, A Playbook for Fostering Hospital-Community Partnerships to Build a Culture of Health.</p><p>Priority needs that the featured hospital-community partnerships are addressing include:</p><ul><li>Chronic disease management</li><li>Health care access</li><li>Mental and behavioral health</li><li>Obesity</li><li>Preventive health practices</li><li>Social determinants of health</li><li>Substance use disorders</li><li>Violence</li></ul><p>Partnerships are important because individuals and organizations share valuable insights, tools and resources. In addition, some individuals are not homeless but live in unstable conditions. Housing instability—a term for the continuum between homelessness and stable, secure housing—can range from physical conditions such as poor sanitation, heating and cooling; exposure to allergens or pests; and substandard housing structures. Unstable access to housing and severe rent burden also contribute to housing instability.</p><h2><small>How is housing instability related to health?</small></h2><p>Where people live and how people live directly affect their well-being. Research shows that individuals experiencing housing instability have limited access to preventive care and are more likely to have infectious diseases and chronic health conditions like diabetes, cardiovascular disease and chronic obstructive pulmonary disease. Homeless individuals also have a shorter lifespan. This guide highlights a web of socio-economic factors that are interconnected, contribute to housing instability and influence health outcomes.</p><h2><small>What is the role of hospitals in addressing housing barriers?</small></h2><p>Hospitals and health systems can implement strategies and programs to improve housing stability, such as neighborhood revitalization, home assessment and repair programs, and care transition programs. This guide recommends a step-by-step pathway for hospitals to consider when developing housing programs. Food insecurity, a determinant of health, affected more than 12.7 percent of U.S. households—that is, 15.8 million households—in 2015.</p><h2><small>How are food insecurity and health related?</small></h2><p>Adults who are food insecure are at an increased risk of developing chronic diseases, and children who are food insecure are at risk for developmental issues. This can lead to an increase in hospital readmissions and medical treatments. This guide describes the link between food insecurity and adverse health issues and outlines clinical and nonclinical approaches that hospitals and health systems can use to reduce food insecurity and the stigma often associated with it.</p><h2><small>What is the role of hospitals in addressing food insecurity?</small></h2><p>Case studies in this guide highlight the innovative strategies and programs of several hospitals and health care systems to reduce food insecurity in their communities.</p><ul><li>Arkansas Children Hospital, in partnership with the U.S. Department of Agriculture, offers free healthy lunches to children on campus and in clinics.</li><li>Boston Medical Center has an on-site food pantry that distributes more than 15,000 pounds of healthy food to referred patients every week.</li><li>Eskenazi Health is partnering with Meals on Wheels of Central Indiana to provide recently discharged patients with 30 days of medically tailored meals for free.</li><li>ProMedica has integrated the Children's HealthWatch Hunger Vital Sign<sup>TM</sup> survey in its database to identify patients who are food insecure. The intersection of community health, population health and population health management – managing the health of a specific patient or clinical population – is expected to bring together multiple sectors. Hospitals and health systems are focusing on community health in new ways as they integrate outreach activities more closely with public health, collaborate with a wide variety of community partners and engage with the community to build an infrastructure for good health. This work acknowledges and emphasizes that investing in ameliorating the social and economic determinants of health will be as important as delivering high-quality medical services.</p> <p>The Association 2016 Committee on Research was charged with exploring what the next generation of community health may look like as hospitals redefine themselves to keep pace with the changing health care landscape.
