But how do they stack up against care delivered in primary care practices and facilities managed by hospitals and health systems?</p> <p>More than 70% of health care leaders globally believe the quality of care in retail settings is lower than in primary care practices, according to <a href="https://catalyst.nejm.org/doi/full/10.1056/CAT.22.0312?query=CTOC&cid=DM1565964_Catalyst_Subscriber&bid=1217533739" target="_blank" title="NEJM Catalyst: Retail Health Care Improves Access, but Quality Concerns Remain">survey findings from NEJM Catalyst Insight Council members</a> published in October. Nearly half of 767 respondents (including 513 from the U.S.) also voiced concerns about the difficulty of tracking patients who use retail clinics over time along with challenges in managing continuity of care.</p> <p>Nevertheless, 66% of survey respondents globally say that the pandemic improved patient views on retail clinics.</p> <h2><span>A Wake-Up Call on Convenience and Better Access</span></h2> <p>Interviews that NEJM Catalyst conducted with respondents provided context on these issues, with Dennis Jolley, system vice president of strategy and planning at UW Health in Madison, Wisconsin, noting that the pandemic provided a wake-up call for providers about the need to improve patient access and convenience.</p> <p>Now that the worst of the pandemic appears to be over, patients who used virtual visits or retail clinics are more comfortable with these options because they have experienced them, Jolley said.</p> <p>Respondents’ concerns about quality are likely related to differences between episodic and longitudinal care strategies, Jolley explained. Retail health tends to be episodic while good primary care is more longitudinal.</p> <p>For health systems, the two biggest benefits of owning or partnering in retail care are to improve access to care (cited by 52% of respondents) and meet consumer demand (48%), an indication of the role that retail care is playing in fulfilling unmet patient needs. Another important finding, respondents say that retail care helps meet the needs of vulnerable populations, with two-thirds (67%) reporting that the use of retail health care delivery has increased access for this group.</p> <h2><span>The Continuity of Care Conundrum</span></h2> <p>Providing continuity of care among retail clinics and primary care practices, hospitals or health systems is a key concern for roughly half the U.S. respondents. The two biggest challenges for health systems of owning or partnering in retail care are the difficulties of tracking patients over time and transitions of care, cited by 46% and 50% of the U.S. respondents, respectively.</p> <p>Some are concerned that preventive screenings may not be done when care is episodic and that retail care will erode the continuity of care and increase the total cost of care in the end. David Fairchild, M.D., chief medical officer for MinuteClinic at CVS, challenged this latter notion in an <a href="https://catalyst.nejm.org/doi/full/10.1056/CAT.22.0311" target="_blank" title="NEJM Catalyst: Growing Acceptance of Retail Health Care">analysis of the survey findings</a>.</p> <p>Fairchild says the quality concerns raised by health care leaders are not necessarily unique to retail care and are found throughout health care delivery. He notes that the field still does not have a common way to share medical information due to interoperability issues and myriad different electronic health record systems.</p> <h2><span>Will Providers Build a Retail Presence?</span></h2> <p>Despite the growth of retail health clinics, only 15% of respondents own or have a formal relationship with these outlets; another 10% say their organization is planning an affiliation within the next three years.</p> <p>And in terms of what they view as the greatest outside competitive threat to traditional health care institutions, more than half the respondents (56%) cited direct-to-consumer telemedicine by a wide margin versus 16% for retail clinics and 13% for urgent care clinics.</p> <p>As for how hospitals and health systems can respond to current trends in retail care, U.S. health care leaders offer four insights.</p> <h3><span>INSIGHT 1</span> <span>|</span> Expand primary care access and new modes of delivering treatment.</h3> <p>Statistics show as many as 25% of Americans do not have a primary care provider. Consider providing an additional access point for consumers who do not have a primary care physician. Be prepared to address care coordination, a potential rise in patients with chronic conditions and the ability to make timely referrals to specialists.</p> <h3><span>INSIGHT 2</span> <span>|</span> Uberize care with GPS-enabled smartphones and intelligent navigation to match immediate demand to care coordination.</h3> <p>Traditional care delivery systems need to re-examine access and ways to make it easier for patients to get the care they want when they want it and where, noted one executive. Meet patients where they are — on their mobile devices, at home, at work or on the road.</p> <h3><span>INSIGHT 3</span> <span>|</span> Consider focusing on in-home care.</h3> <p>More than 40% of U.S. respondents say that in-home care will reduce demand for retail care. But as we’ve seen with other retailers getting involved in health care, a growing number are looking to expand operations into home care. Be prepared to differentiate your services.</p> <h3><span>INSIGHT 4</span> <span>|</span> Partner and coexist with retail care.</h3> <p>Traditional practices and organizations often are not nimble enough to meet same-day, urgent care needs for basic acute care like treating strep throat, noted one CMO from a U.S. nonprofit research facility. Providers must recognize changing consumer preferences in how they want to access care, build diverse care teams and prioritize care continuity, notes a recent Deloitte report, <a href="https://www2.deloitte.com/us/en/insights/industry/health-care/alternative-sites-of-care.html" target="_blank" title="Deloitte: Advancing health through alternative sites of care">“Advancing Health Through Alternative Sites of Care.”</a></p> <h2><span>What Consumers Want</span></h2> <p>The report zeroes in on how to create a more inclusive and equitable experience, based in part on two surveys Deloitte conducted in February and March among more than 4,500 consumers and online focus groups with 445 consumers who identify as Black, Hispanic, Asian or Native American.</p> <p><strong>Among the findings:</strong></p> <ul> <li> <h3><span>Reduce Friction</span></h3> <p>Health care organizations must provide a patient experience that reduces friction and addresses factors that contribute to inequitable care and poor access.</p> </li> <li> <h3><span>Touch Points</span></h3> <p>Alternative care sites can increase accessibility by providing more patient touch points and expanding location options for well visits, mental health and other services.</p> </li> <li> <h3><span>Retail Clinics</span></h3> <p>While only about 10% of consumers have used a retail clinic over the past year, many more say they would be “likely to” or “maybe would” use these locations for preventive care (55%) or mental health (47%). Black, Asian and Hispanic respondents are more likely than white respondents to use retail clinics, and urban residents are more likely than those in rural areas to use retail clinics.</p> </li> <li> <h3><span>Virtual Care</span></h3> <p>Consumers’ use of virtual health is shifting from just urgent or acute care to include preventive and mental health. Nearly two-thirds — across all races and ethnicities — say they would use virtual visits for preventive care while nearly 75% of Medicaid and health insurance exchange plan users would access care virtually for mental health services.</p> </li> </ul> <h2><span>Addressing the Trust Deficit</span></h2> <p>To reach more of these consumers through alternative care sites, providers must develop diverse care teams, Jay Bhatt, D.O., managing director of Deloitte Services LP and one of the report’s co-authors, told <a href="https://medcitynews.com/2022/10/alternative-care-sites-wont-make-a-difference-unless-these-2-priorities-arent-considered/" target="_blank" title="MedCity News: Alternative sites won't make a difference unless these 2 priorities are considered">MedCity News</a>. “Having diverse and empathetic care teams that reflect the communities they serve will help consumers build and sustain trust. If you meet community members where they live, work and play, they will feel more comfortable,” Bhatt said.</p> <p>Trust deficits remain a major barrier for people seeking care, the report notes. Consumer and community trust in providers and organizations is critical for optimal health because trust influences patients’ willingness to receive preventive screenings and physical and mental health care. Trust between patients and providers also is linked to improved patient experiences, better health outcomes and the patients’ perceptions of the care they receive, Deloitte research shows.</p> <p><strong>In its 2021 focus groups, Deloitte found that:</strong></p> <ul> <li>Two of every three participants who identify as Black, and half of Asian and Hispanic participants prefer to see a provider who is similar in race, background or lived experience.</li> <li>For Asian (59%) and Hispanic (53%) participants, having a provider who is empathetic and culturally competent is a top priority.</li> <li>About half of participants are willing to trade access to convenient, in-person care for a virtual visit with a provider who looks like them and is culturally competent.</li> </ul> <h2><span>5 Takeaways for Health Care Organizations</span></h2> <h3><span>1</span> <span>|</span> Create more access points and include opportunities to address drivers of health.</h3> <p>Organizations have an opportunity to address the issues that exacerbate disparities. Retail clinics and community health centers provide better access to care as well as access to food, educational resources and connections to social service agencies.</p> <h3><span>2</span> <span>|</span> Develop diverse care teams.</h3> <p>Consumers are most likely to be comfortable receiving care from someone who is “like them.” Consider co-creating teams with the community to reflect what the community wants.</p> <h3><span>3</span> <span>|</span> Ensure care continuity.</h3> <p>For patients with chronic conditions or older adults who require more complex care, retail clinics can focus on seamless interactions with other health systems or the primary care setting. Provide an easy mechanism to send the patient’s visit details to the entire care team.</p> <h3><span>4</span> <span>|</span> Invest in virtual health technology.</h3> <p>Improving telehealth capabilities and designing a process whereby consumers can access their own physician instead of third-party services can help health care organizations streamline and maximize virtual health benefits.</p> <h3><span>5</span> <span>|</span> Train clinicians on virtual health.</h3> <p>Patients still aren’t completely satisfied with their virtual interactions with physicians and other clinicians. Rapids, Mich.-based Spectrum Health makes remote consultations available via mobile carts in 34 medical specialties, and Columbus (Neb.) Community Hospital employs interdisciplinary teams to strengthen communications with providers inside and outside of the hospital. Read these AHA Value Initiative case studies to learn more.</em></p> <h2>Saving time and money through telehealth services</h2> <p>In 2014 Spectrum Health, Grand Rapids, Mich., created its telehealth program in an effort to reduce patient transfers, increase access to care and reduce costs. The program serves the system’s 15 hospitals and 180 ambulatory sites throughout Michigan.</p> <p>The highest volume of services is provided to patients with behavioral health, sleep, wound care and infectious disease conditions. A site-to-site specialty telehealth program called Specialty MedNow makes consults available via mobile carts in 34 medical specialties.</p> <p>Telehealth services have now become a standard of care at Spectrum. As of November 2018, the organization has avoided 323 inpatient transfers, saved $5.9 million and served 13,772 new primary care patients. </p> <p>Learn more about the program <a href="/case-studies/2019-01-29-aha-value-initiative-members-action-spectrum-health-grand-rapids-mi-0">here</a>.</p> <h2>Strengthening continuity of care with shared processes</h2> <p>Through building relationships among providers across the care continuum and implementing standardized processes, Columbus (Neb.) Community Hospital decreased its avoidable readmission rates and enhanced quality of care. Interdisciplinary teams work with skilled nursing facilities, home health agencies, assisted living facilities, retail pharmacies and medical groups to strengthen care plans and streamline paperwork.</p> <p>Case managers, hospitalists, social workers and others huddle twice daily to anticipate inpatient needs and conduct risk assessments for patients more likely to be readmitted. Team members adhere to standardized processes for care, medication reconciliation and discharge planning to ensure continuity from hospitalization to outpatient care to home. </p> <p>From October 2016 to May 2018, the hospital reduced all-cause 30-day readmissions by 42 percent and saved $819,797 in hospital care.</p> <p>Learn more about the program <a href="/case-studies/2019-01-29-aha-value-initiative-members-action-columbus-community-hospital-columbus-ne">here</a>.<br />  </p> Tue, 29 Jan 2019 10:00:06 -0600 Care Continuum House Holds Hearing on Consolidation in Health Care Sector /news/headline/2018-02-14-house-holds-hearing-consolidation-health-care-sector <p>The House Energy and Commerce Subcommittee on Oversight and Investigations today held a <a href="https://energycommerce.house.gov/hearings/examining-impact-health-care-consolidation/">hearing</a> examining the impact of health care consolidation, which featured academic witnesses. In a <a href="/letter/2018-02-14-aha-energy-and-commerce-subcommittee-oversight-and-investigations-leaders-hearing">letter</a> submitted to the committee, AHA Executive Vice President Tom Nickels said, “If there was ever any doubt that the health care landscape is changing dramatically, one has to look no further than the recent announcements that CVS seeks to acquire Aetna; Amazon, Berkshire Hathaway and JP Morgan Chase & Co partnering on employee health care; and UnitedHealth Group embarking on a physician acquisition spree. The hospital field has long anticipated this disruption and has sought to participate by replacing an outdated ‘siloed’ health care system with a continuum of care to improve coordination and quality and reduce costs for patients. Building a continuum demands that providers are more integrated.
