The bipartisan legislation extends the program for three years and would increase current state allocations from 30 to 35 physicians per year. Over 80 million Americans live in areas that lack access to a primary care physician, and disparities across health specialties also continue to grow. For example, one-third of Black Americans live in areas that lack sufficient access to a cardiologist. We are committed to promoting access to preventive medicine that keeps patients healthy and able to manage their health conditions. Unfortunately, without sufficient access today, more Americans will face worse health outcomes because they lack access to health care in their communities.</p><p>By 2036, the United States will face a shortage of up to 86,000 physicians. In that same time, there will be significant growth in the number of Americans aged 65 and older who will need more health care and access to more types of physicians. The growing demands on health care systems require Congress to take steps to prepare to meet the needs of all patients. We continue to advocate for increasing the number of graduate medical education slots, minimizing administrative burdens on physicians and practices, and recognizing the vital role international medical graduates play in our health care system.</p><p>A growing number of physicians are nearing the traditional retirement age or will over the next decade. While investments in GME will help to mitigate some physician shortages, it could take up to 15 years before a physician is educated, trained, and seeing patients. As a result, increasing the total number of Medicare supported GME slots cannot be the only solution to addressing shortages, particularly in underserved areas.</p><p>That is why we support strengthening the Conrad 30 waiver program. For 30 years, the Conrad 30 waiver program has incentivized approximately 20,000 highly skilled physicians to practice medicine in rural and underserved areas. A key part of the success of the current program is its flexible design that allows each state to customize how it allocates its 30 waivers to respond to its unique needs.</p><p>Without a Conrad 30 waiver, IMG physicians on a J-1 visa are required to return to their country of origin for at least two years before applying for another visa or green card. In exchange for the waiver, they must practice in a federally designated underserved area for at least three years.</p><p>Reauthorizing this program would provide critical updates to the program. In addition to gradually increasing the number of available waivers per state if certain thresholds are met, it would clarify and improve the waiver process for IMGs and employers by expanding the timeframe between when individual receives a waiver and when they must begin work to allow for time to complete their medical education and receive employment authorization.</p><p>These overdue changes will make it easier for employers to recruit and retain physicians who are practicing in underserved areas. It also clarifies the incentives for physicians to pursue a waiver, who would better understand the waiver process and employment obligations.</p><p>The Healthcare Workforce Resilience Act would initiate a one-time recapture of up to 40,000 unused employment-based visas – 25,000 for foreign-born nurses and 15,000 for foreign-born physicians – so they can strengthen and provide stability to the U.S. health care system. This legislation would allow for thousands of international physicians who are currently working in this country on temporary visas with approved immigrant petitions to adjust their status. This crucial policy change, which concludes three years after the date of enactment, will enable these physicians to continue serving patients ensuring every American can access needed care.</p><p>Foreign-born physicians are an invaluable component of the U.S. health care system, comprising nearly 1 in 5 of active U.S. physicians. H-1B physicians practicing vital specialties like geriatric medicine and nephrology also make up approximately 50 percent of active physicians. These realities should compel Congress to immediately take steps to address health workforce shortages and strengthen the highly successful Conrad 30 program that has incentivized U.S.-trained foreign physicians to rural and underserved areas for 30 years.</p><p>Thank you for your consideration. September 10 was <strong>World Suicide Prevention Day</strong> and September 17 is <strong><a href="https://npsaday.org/" title="National Physician Suicide Awareness Day">National Physician Suicide Awareness Day</a></strong>.</p> <p>A recent <a href="https://suicidepreventionnow.org/static/executive-summary-2022-9c5a59e0f8016f1803570b11cfd3cb29.pdf" title="Suicide Prevention Now | Public Perception of Mental Health and Suicide Prevention Survey Results">public poll</a> suggests that mental health has become a leading priority in the U.S., with 76% of people saying their own mental health is equally important to physical health. More than 8 in 10 adults (84%) say that as a result of the pandemic, it's more important than ever to make suicide prevention a <a href="https://suicidepreventionnow.org/" title="Suicide Prevention Now">national priority</a>. Public sentiment that suicide can and should be prevented is strong, and help is available 24/7 by calling <a href="tel:988" title="988 Suicide & Crisis Lifeline">988</a>, the <a href="https://988lifeline.org/" title="988 Suicide & Crisis Lifeline">Suicide and Crisis Lifeline</a>.</p> <p>However, real or perceived access to quality suicide-specific care is limited for some. When I talk about suicide with health care professionals, I often start by asking for a show of hands: “How many people would bring their loved one who was having thoughts of suicide or needed treatment for suicidality to the health care system in which you work?” Typically, far less than half the hands in the room go up. Given this, it is not surprising that many health care providers frequently bury or ignore their own thoughts of suicide. In addition to perhaps questioning whether effective treatment options exist (they do!), many providers also don’t attend to their own thoughts of suicide out of fear over what their colleagues, supervisors or patients might think or the repercussions that could happen – whether those repercussions are real or just perceived.</p> <p>Along with individual action, the systems in which health care providers work have a role to play in suicide prevention. It is the responsibility of health care leadership to ensure that they build a restorative and just culture where taking care of oneself is expected, respected and modeled, and where access to quality, effective treatment for suicide is available. The Suicide Prevention in the Health Care Workforce guide, produced by the AHA and the CDC, is a resource that offers hospitals and health systems proven strategies to improve the mental health and well-being of their staff. Evidence-based practices for suicide do exist and, when used with fidelity, health care systems are substantially reducing suicides for patients in their care. Health care systems that have adopted Zero Suicide typically find that the program adds neither labor nor cost; in fact, most systems find it to be a more efficient use of resources. Visit <a href="https://zerosuicide.edc.org/" title="Zero Suicide">Zero Suicide</a> for more information on best practices for health care systems.</p> <p>During Suicide Prevention Month, make it a point to check in on your colleagues, friends, patients, as well as yourself. Ask directly when you are concerned about suicide risk (“Are you thinking about killing yourself?”) and provide support (check in regularly with them). Health care leadership should work to eliminate stigma around seeking care; ensure access to quality treatment is available when and how it is needed; and that job stressors are acknowledged and reduced.</p> <p>Flight attendants tell parents to put on their own oxygen masks before assisting others, even though every parent’s first instinct is to protect their child over themselves. Health care workers share this propensity, which can make them invaluable caregivers – but also vulnerable to ignoring their own health and well-being. Know that your strength and capacity as a health care provider grows stronger when you put your oxygen mask on first. However, hospitals and health systems now face mounting and critical physician shortages — estimated to reach 124,000 physicians by 2033 — that will jeopardize access to care in communities across the nation.</p> <p>These shortages — combined with an aging population, a rise in chronic diseases and behavioral health conditions, physician burnout from the pandemic, and “state-of-the-art” care delivery advancements — all contribute to a need for robust graduate medical education (GME) funding to adequately prepare America’s health care workforce for the health system of the future and ensure continued access to care.</p> <p>Congress enacted the Medicare GME program to ensure a sufficient supply of well-trained physicians. However, the current cap on Medicare-supported residency positions, imposed by the Balanced Budget Act of 1997, restricts the number of positions for which teaching hospitals can receive direct GME funding (DGME) and the amount of indirect graduate medical education (IME) funding they receive.</p> <h2>AHA Take</h2> <p><strong>We urge Congress to increase the number of Medicare-supported GME residency positions.</strong></p> <p>The AHA supports the bipartisan Resident Physician Shortage Reduction Act of 2023 (H.R. 2389/S.1302) which would add14,000 Medicare-funded residency slots over the next seven years. The bill would also require the Government Accountability Office to study strategies for increasing the diversity of the health professional workforce and report its findings and recommendations to Congress within two years of enactment.</p> <h2>Background</h2> <p>Physician shortages are growing, exacerbated by caps on the number of Medicare-funded residency slots. The Association of American Medical Colleges projects a national shortage of up to 124,000 physicians by 2033, including shortages of primary care physicians and specialists, such as pathologists, neurologists, radiologists and psychiatrists.</p> <p>While the aging of the U.S. population and the physician workforce drive some of the projected shortage, much of it stems from the caps on Medicare-funded residency slots imposed by Congress nearly 25 years ago as a cost-saving measure, based on erroneous projections about the need for physicians. Although the number of medical school graduates has increased significantly over the past two decades, Medicare-funded training opportunities for these graduates has remained frozen at 1996 levels. As a result, more than 3,300 applicants lacked residency slots in 2022.<sup>1 </sup>Furthermore, the caps have created imbalances that favor allocation of slots toward lower-cost and higher-reimbursement specialties, rather than more urgently needed primary care and behavioral health. While some hospitals are filling in gaps by self- funding a portion of their own, this model is not sustainable over the long haul, as evidenced by the -17.3% Medicare margins (without COVID-19 relief funds) for teaching-only hospitals in 2020.<sup>2</sup></p> <p>Lifting the cap on Medicare-funded residency positions would enhance access to care and help America’s hospitals better meet the needs of the communities they serve. Increasing Medicare-funded residency slots would provide hospitals more flexibility to diversify and maintain training programs, including both primary care and specialty programs. In addition, an increase in slots would allow health systems to train residents in more diverse facility types, such as smaller rural hospitals which may not be able to operate their own training programs. Your bipartisan bill would increase by 14,000 the number of Medicare-funded residency positions, thereby helping to alleviate the physician shortages that threaten patients’ access to quality care.</p> <p>In creating the graduate medical education (GME) program, Congress acted to ensure an adequate supply of well-trained physicians. However, the cap on residency slots established in the Balanced Budget Act restricts the number of residency slots for which hospitals may receive direct GME funding. The cap also limits the number of residents that hospitals may include in their ratios of residents-to-beds, which affects indirect medical education (IME) payments to teaching hospitals.</p> <p>Congress added 1,000 new Medicare-supported GME positions at the end of 2020 and 200 additional positions in 2022 – the first increases of their kind in more than 25 years. Your legislation would build on Congress’ historic investment by increasing the number of Medicare supported residency positions by 2,000 each year for seven years, for a total of 14,000 new slots. A portion of these positions would be targeted for hospitals already training over their caps, hospitals in rural areas, hospitals in states with new medical schools or branch campuses, and hospitals serving areas designated as health professional shortage areas.</p> <p>Your legislation responsibly addresses the nation’s urgent need for additional physicians. In addition to proposing a 3.1% increase in inpatient PPS payments for 2021, the rule would require disclosure of certain payer-negotiated rates and makes changes to Disproportionate Share Hospital (DSH) payments, Chimeric Antigen Receptor T-cell (CAR T) therapy payment and quality incentive programs.</p> <h2><span>AHA Position</span></h2> <p>Congress should take a number of actions to help
