Founded as City Hospital in 1860 to serve the health care needs of the rapidly developing city, Denver Health has since grown alongside the community to become a complete health care system.</p> <p>Denver Health provides health care services to one in three residents of the metropolitan area. Many of its patients have complex health care and social needs, including the uninsured, people experiencing poverty, people living with substance use disorder, pregnant teens, victims of violence, people experiencing homelessness, and victims of trauma. The health system delivers preventive care, primary care, and acute care services, with a focus on integration to ensure the best, most cost-effective care possible. Denver Health provides hospital and emergency care to the public regardless of ability to pay, providing hundreds of millions of dollars in uncompensated care annually.</p> <h2><span>Patient–Payer Mix</span></h2> <p>As Colorado’s primary metropolitan anchor hospital (MAH), Denver Health faces a challenging patient-payer mix. More than 80% of Denver Health’s patient visits and 83% of discharges are individuals with Medicare, Medicaid, or without any insurance.</p> <p>Further, revenues from these same patient groups represent 80% of Denver Health’s gross patient revenue. In 2022, Denver Health incurred $120 million of uncompensated care cost — for which the city's annual fixed payment of $29.7 million covered less than 25%. According to the Colorado Department of Health Care Policy and Financing (HCPF) 2023 Hospital Expenditure Report, Denver Health's “Percentage of Total Care for Uninsured” was 15.9%––significantly greater than other hospitals in the Front Range region. Like other MAHs, Denver Health faces significant financial challenges caused by low margins and workforce shortages, both of which have been exacerbated by the COVID-19 pandemic.</p> <h2><span>Denver Health's Unique Approach to Care</span></h2> <p>Despite these challenges, Denver Health has found innovative ways to serve the community and holistically care for patients.</p> <h3><span>Addressing Social Determinants of Health</span></h3> <p>Denver Health recognizes that many of the care needs addressed in its hospital system are linked to social determinants of health, such as neighborhood safety, systemic racism, housing, food security, and limited transportation. Denver Health works closely with the City and County of Denver, as well as its civic, business, and nonprofit partners, to identify and address community needs through a range of programs and initiatives. One recent example is a partnership with the Denver Housing Authority to develop a transitional housing unit so patients experiencing homelessness have a place to heal and recover from the hospital.</p> <h3><span>Finding Innovative Ways to Reach Vulnerable Communities</span></h3> <p>Denver Health is constantly finding new ways to reach historically marginalized community members. For example, the recently launched Mobile Health Centers (MHCs) have been an invaluable tool in efforts to bring COVID-19 testing and vaccinations into some of Denver’s hardest-hit neighborhoods and deliver care to people who otherwise might not have access. Once COVID-19 efforts are no longer necessary, the MHCs will be used to deliver other primary care services, including important health screenings.</p> <h3><span>Serving as an “Anchor Institution”</span></h3> <p>Denver Health seeks to leverage its economic power alongside its human and intellectual resources to improve the long-term health and social well-being of its community. Recent efforts include:</p> <ul> <li>Establishing a Workforce Development Center to provide low-income employees with financial education and individualized counseling, career pathing, and access to social services.</li> <li>A partnership with Emily Griffith Technical College to manage the OMC Café, providing students with employment while helping them learn new skills that can lead to a career.</li> <li>Hiring a local, minority-owned real estate firm to handle the sale of Denver Health properties.</li> <li>Starting a supportive enrollment process to help families enroll in SNAP, WIC, and other programs they may qualify for, with the goal of helping to stabilize families and make sure children can flourish.</li> <li>Providing job opportunities with growth potential by partnering with local community organizations to place people in positions at Denver Health.</li> </ul> <h2><span>Metropolitan Anchor Hospitals Need Greater Support</span></h2> <p>Despite the financial challenges MAHs face, Denver Health has continued to expand programs, increase patient visits, and develop innovative ways to provide care. But to be more impactful, MAHs like Denver Health need adequate and sustained federal support. Founded in 1962, University Health is an academic medical center providing accessible, state-of-the-art quality health care to the Kansas City community without regard for whether one has insurance or not. With two acute care hospitals and more than 70 primary and specialty clinics, University Health employs more than 4,500 highly trained individuals.</p><p>Affiliated with the University of Missouri-Kansas City (UMKC) Schools of Medicine, Nursing, Dentistry, and Pharmacy, University Health trains the new generation of health care providers by providing innovative instruction to more than 600 medical students and 350 resident physicians annually, and by offering cutting-edge care to all patients. University Health is home to one of only two adult Level 1 Trauma Centers in the city; the area’s largest behavioral health program; and, the only freestanding specialty and surgery center in downtown Kansas City. A new women's and primary care health center opened in late 2020. University Health delivers nearly half of the babies born in Kansas City, Missouri. University Health also operates the Jackson County Health Department.</p><p>In addition to its hospital facilities, University Health has primary care clinics throughout Jackson County. University Health also brings health care to the community via mobile clinics where providers perform health screenings, share health information, and provide testing and vaccines, as well as distribute fresh fruits and vegetables to schools and other sites around the community.</p><blockquote><h3><span><em>"University Health is an academic health center providing accessible, state-of-the-art quality healthcare to our community regardless of the ability to pay.”</em></span></h3><p>University Health’s Mission Statement</p></blockquote><h2><span>Patient–Payer Mix</span></h2><p>University Health’s patients have a variety of medical and socioeconomic needs. Because Kansas City has a substantial population of immigrants and refugees, many University Health patients need interpretation services and culturally competent care that respects personal values. As a result, University Health provides its clinical and nonclinical staff with cultural competency-based training and development. In 2021, the system provided more than 224,000 interpretation services in more than 100 languages, including Arabic, Somali, Spanish, Vietnamese, and American Sign Language.</p><p>As a metropolitan anchor hospital (MAH), about half of University Health’s patient population comes from historically marginalized communities; most patients are uninsured or underinsured. Together, Medicaid and Medicare make up 72% of all visits, 59.7% of all discharges, and 63.3% of net patient revenue. A relatively small percentage of patients have commercial insurance. Last year, University Health provided nearly $140 million in uncompensated care. MAHs like University Health face significant financial challenges, low margins, and workforce shortages—all of which have been greatly exacerbated by the COVID-19 pandemic. Yet despite these challenges, University Health continues to provide more uncompensated care than all other hospitals combined in Kansas City, as well as the most uncompensated care in the state of Missouri per capita.</p><h2><span>University Health's Unique Approach to Care</span></h2><p>To achieve its mission of providing accessible, state-of-the-art quality health care regardless of patients’ ability to pay, University Health leadership has worked to:</p><h3><span>Engage with the Community</span></h3><p>University Health maintains robust partnerships throughout the Kansas City community. For example, the Community Health Strategy & Innovation team finds new ways to educate Kansas City residents and promote long-term solutions to maintain a healthier lifestyle. Using a wellness connection model, community health nurses deliver preventive services in multiple locations. University Health also is proud to provide evidence-based programs designed to address social determinants, such as health education and access to care.</p><p>Recognizing the social and economic factors that influence health care outcomes, University Health utilizes cultural health navigators with its patients and operates the Healthy Harvest Mobile Market; a trauma-informed care education program; and, a nutritional education program. The system employs more than 40 social workers, connects the community with social services, including housing assistance, and maintains relationships with area transportation authorities to make it easier for patients to access the care they need. University Health also operates Missouri’s largest WIC program.</p><h3><span>Foster a Culture of Diversity, Equity, and Inclusion</span></h3><p>University Health believes it is essential to maintain a culture that values diversity, equity, and inclusion. This is evidenced by ranking first in Missouri and eighth in United States on the Lown Institute Hospital Index for Social Responsibility’s racial inclusivity ranking; ranking sixth on Forbes Magazine’s Best Employers for Diversity list in the Health and Social category; and, maintaining Leader status on the Human Rights Campaign’s Healthcare Equality Index for five consecutive years. To build a workforce that reflects the communities it serves, University Health has enhanced physician and provider recruitment at Historically Black Colleges and Universities and Hispanic Serving Institutions. In addition, the University Health Supplier Diversity Program works with historically underused businesses, such as Minority Owned Business Enterprises and Women Owned Business Enterprises, service-disabled veterans, and representatives of the LGBTQ+ community.</p><h2><span>Metropolitan Anchor Hospitals Need Support</span></h2><p>Despite the financial challenges MAHs face, University Health continues to support innovative programs to improve the health and wellbeing of the Kansas City community. University Health hopes to intensify its efforts, especially by strengthening its workforce and expanding the care it provides to its growing, diverse population. University Health, like all MAHs, must have more stable funding and resources. In operation since 1931, UMC is also the oldest hospital in the region, and it strives to provide patient-centered care in a fiscally responsible and learning-focused environment.</p> <p>UMC is the only hospital in southern Nevada with a Level 1 trauma center and transplant center. In addition, UMC sets itself apart by maintaining an accredited children’s hospital, a Ryan White-accredited HIV/AIDS clinic, and one of the nation’s 64 verified burn centers.</p> <p>A partnership with the University of Nevada, Las Vegas (UNLV) Medical School has also helped establish UMC as a leading academic medical center. UMC’s robust ambulatory division provides primary and urgent care across 15 clinics throughout the valley and serves between 300,000 and 400,000 patients per year.</p> <h2><span>Patient–Payer Mix</span></h2> <p>Nevada, as a whole, has a large Medicaid population and a high unemployment rate, primarily due to reduced tourism resulting from the COVID-19 pandemic. UMC’s patient-payer breakdown is approximately 48% Medicaid, 18% Medicare, 19% commercially insured, 10% uninsured, and 5% governmental. Two-thirds (66%) of all visits are by Medicaid or Medicare beneficiaries, which account for 72.5% of UMC’s patient discharges. Nearly three-quarters (72%) of UMC’s net patient revenue is Medicaid and Medicare revenue. Each year, UMC typically provides an estimated $40.9 million in uncompensated care.</p> <p>In the last decade, UMC has been able to reinvest its profits back into the hospital system and become a leader in the area, thanks largely to its fiscal discipline. This reinvestment has largely been directed towards facility improvements and human capital like doctors and surgeons, which has allowed UMC to compete with private hospitals. The success of UMC can be credited largely to:</p> <h3><span>Community Outreach</span></h3> <p>UMC’s Healthy Living Institute sponsors meetings with schools to talk about health and safety, hosts new parent education sessions, and runs “daddy boot camps” for new fathers. Other programs include exercise classes for seniors to reduce falls and babysitter-certification classes that teach CPR and choking interventions. UMC also hosts health fairs and a variety of other safety education services.</p> <h3><span>Employee Engagement and Investment</span></h3> <p>UMC has achieved an outstanding employee retention rate by striving to make all employees know their contributions are valued. The average tenure for staff is 10 years, but many have worked for UMC for much longer.</p> <h3><span>Commitment to Caring for HIV/AIDS Patients</span></h3> <p>UMC has had a long-standing and proud commitment to caring for HIV/AIDS patients through the UMC Wellness Center. In fact, UMC pioneered HIV/AIDS care in Nevada and continues to lead the way. The Wellness Center offers on-site Ryan White eligibility services as well as case management, social worker, nutritional therapy, and mental health services.</p> <h3><span>Life-Saving Trauma Care</span></h3> <p>Offering the highest level of trauma care in the state of Nevada, UMC is a core component of Nevada's emergency medical system. The level of care for injured patients is continuously pursued through clinical care services, active research and education, and prevention programs. With a service area covering 10,000 square miles including Southern Nevada, parts of California, Utah, and Arizona, the trauma center serves the two million residents of Clark County and the 32 million visitors who come to the area each year.</p> <h3><span>COVID-19 Support</span></h3> <p>UMC has administered nearly 2 million COVID-19 PCR tests, helping to fill much-needed gaps in the community. The hospital has also helped essential workers in the Las Vegas area feel more comfortable returning to work by handing out personal protective equipment (PPE). The hospital has partnered with a local casino to create a vaccination site and has administered an estimated 100,000 vaccines. UMC has also run post-COVID recovery clinics and provided monoclonal antibody clinics in outpatient settings, which have treated approximately 1,000 patients with a 100% survival rate.</p> <h2><span>Metropolitan Anchor Hospitals Need Greater Support</span></h2> <p>Despite the financial challenges that UMC faces, it has found ways to continue serving and supporting the Las Vegas community. But in order to continue this impactful work, including investment in critical infrastructure, staff, and care services, MAHs like UMC need additional financial support.</p> <p>Increased funding would allow UMC and other MAHs to boost capacity, add beds, and make needed infrastructure and overhead improvements. As the chief academic teaching hospital of the Lewis Katz School of Medicine at Temple University, Temple University Hospital (TUH) provides medical education to the next generation of physicians and biomedical scientists, while also serving the broader Philadelphia community through over 600,000 outpatient visits, 151,000 emergency department visits, 10,000 psychiatric crisis response visits, and 17,000 inpatient and outpatient surgeries in 2021.</p><p>The North Philadelphia community is one of the lowest-income areas in Pennsylvania, so the population TUH serves is already dealing with challenges due to social or economic disadvantages. Approximately 62% of the local population has an education level of high school or less, 12% do not speak English as a primary language, and 45% live in households that earn less than the federal poverty level. Of Philadelphia’s 46 neighborhoods, Temple University Hospital’s immediate neighborhood has the lowest life expectancy. With respect to patients served, about 70% have one or more chronic conditions, 50% have a behavioral health diagnosis, and 25% have a substance use disorder.</p><blockquote><h3><span><em>“We face additional pressures on top of the traditional care model that others don’t have. Young</strong></span><br>President and Chief Executive Officer, TUH</p></blockquote><h2><span>Patient–Payer Mix</span></h2><p>As a metropolitan anchor hospital (MAH), TUH treats some of the most medically complex patients in the region who rely on public programs for health care coverage. Approximately 45% of patients are covered by Medicaid, 41% are covered by Medicare, and 12% are covered by commercial insurance. Medicaid and Medicare patients comprise about 87% of total discharges. Payments from these government payers comprise about 75% of TUH’s net patient revenue. TUH provides about $25 million in charity care each year.</p><p>Due to its patient-payer mix, TUH faces significant financial and workforce challenges. Hiring and retaining the staff necessary to treat this community are two of the biggest challenges facing TUH. Given the overall economics, it is difficult for TUH to offer competitive salaries <em>and</em> provide for the unique needs of the broader community.</p><h2><span>TUH's Unique Approach to Care</span></h2><p>Each year, TUH spends $1.6 billion to further its health care mission. With 50% of its employees living in the community, TUH is a vital part of the local economy. TUH’s contributions include:</p><h3><span>COVID-19 Community Outreach</span></h3><p>TUH administered more than 128,000 COVID-19 vaccine doses, and two-thirds of those recipients were Black or Hispanic. The hospital established a monoclonal antibody infusion clinic and post-COVID-19 recovery clinic. Although TUH’s population is at high risk for COVID-19, its COVID-19 patient mortality rate was 33% better than hospitals across the nation. It was the only Pennsylvania hospital honored by Healthgrades as a <em>Leading Hospital in COVID Care</em> for excellence in treating high volumes of coronavirus patients during the pandemic’s first wave.</p><h3><span>Supporting Moms and Newborns</span></h3><p>TUH’s pre- and post-natal care programs include perinatal care, breastfeeding support, and Sleep Awareness Family Education. TUH offers trauma-informed care for mothers with substance use disorder (SUD), including behavioral health services and medication-assisted treatment. To strengthen equity in care delivery, TUH is planning a dedicated hospital for women and infants. Medicaid covers 90% of infant deliveries.</p><h3><span>Workforce Development</span></h3><p>With more than 10,000 faculty members and employees, Temple University Health System is a major regional employer. More than 2,500 students of medicine, nursing, and other health professions received hands-on training at TUH last year. Trainees are enrolled not only in Temple’s affiliated School of Medicine, College of Public Health, and School of Pharmacy, but in numerous other colleges and community-based training programs.</p><h3><span>Housing Smart</span></h3><p>In collaboration with managed care plans and social service agencies, TUH launched a two-year program to help 25 homeless Medicaid patients who frequently use hospital emergency departments. Patients are provided free housing and caseworkers to connect them with healthcare, nutritious meals, and social services.</p><h3><span>Other Community and Social Support Programs</span></h3><p>Like many other metropolitan anchor hospitals, TUH offers food, housing, behavioral health, trauma, and SUD support programs. A multi-visit patient clinic has been established to specifically treat patients with a high frequency of emergency department use. This holistic treatment of high-use patients not only lowers treatment costs but also leads to better health outcomes.</p><h2><span>Metropolitan Anchor Hospitals Need Support</span></h2><p>Despite its financial challenges, TUH continues to support innovative programs to improve the health and well-being of its community. However, with higher care costs and less reimbursement, staff retention remains a challenge. In addition, SBH Health System maintains a Level 2 Trauma Center authorized to treat critically ill and severely injured patients and serves as a New York State-designated Stroke Center and State-designated AIDS Center. Affiliated with the CUNY School of Medicine, New York Institute of Technology College of Osteopathic Medicine, and the Albert Einstein College of Medicine, SBH Health System trains over 300 residents and fellows annually in a variety of disciplines, including internal medicine, pediatrics, psychiatry, surgery, pharmacy, and dentistry.</p> <p>SBH Health System is a major provider of ambulatory care services in locations throughout the Bronx, with more than 400,000 outpatient visits annually for primary and specialty care. The patients SBH Health System cares for are predominantly low-income, high-need, multilingual, Latinx, and Black individuals and families who live within a mile of the hospital. The neighborhood in which SBH Health System is located is in a community in flux. It continually welcomes new immigrants–while gentrification threatens the ability of many current low-income residents to remain in their homes. <span><strong>Ninety-five percent of care is covered by government payers, including 88% by Medicaid.</strong></span> Medicaid and Medicare beneficiaries make up 96.9% of all annual visits, 60.8% of patient discharges, and 63.3% of net patient revenue. Each year, SBH Health System provides more than $56 million in uncompensated care. Due to this unique payer mix, MAHs like SBH Health System face significant financial challenges, low margins, and workforce shortages—all of which have been exacerbated by the COVID-19 pandemic.</p> <h2><span>SBH Health System’s Unique Approach to Care</span></h2> <p>SBH Health System has a long history of working to eliminate health inequities with the limited resources it has.</p> <h3><span>Demonstrating Commitment to the Local Community</span></h3> <p>SBH Health System provides hyperlocal care in a city filled with world-class hospitals. With most of their patients living within a one-mile radius of the main facility, SBH Health System is truly a community hospital that strives to serve everyone, regardless of payment status. The system is also a bastion of the local economy. Nearly half of SBH Health System employees live in the community, of which 82% are non-white, representing the diversity of the Bronx. The system operates volunteer programs that provide a pathway to employment. For example, one employee began her SBH Health System career as a volunteer who folded baby blankets in the laundry and is now employed as a clerk in the ICU and is enrolled in a master’s degree program. During the pandemic, the system augmented its support for staff by providing groceries and PPE for their families. SBH Health System’s support for its community extends beyond the hospital doors.</p> <h3><span>Looking Beyond Urgent & Emergent Care to Address Equity</span></h3> <p>SBH Health System operates a network of outpatient departments to help address patients’ chronic health needs–including behavioral health–closer to home. In addition, the system opened a new SBH Health and Wellness Center in 2020, which includes the SBH Teaching Kitchen, a gym that is open to the community, centers for Women’s Imaging, Children’s Health and Women’s Health, a WIC site, and a rooftop farm with beehives that helps to stock a food “farmacy.” In a short time, the services have already made a measurable difference in the health and wellness of the community, and the site is serving as a “social determinants laboratory” allowing them to implement pilot projects rapidly. For example, SBH Health System is running integrated programs for individuals with obesity and type 2 diabetes, successfully helping participants decrease BMI and improve blood glucose stabilization. SBH Health System also distributes bags of produce grown on the roof-top farm to high-risk patients discharged from the hospital to ensure they have enough food in their homes. Moreover, SBH Health System maintains strong relationships with community groups to mitigate violence through trauma-informed care. For example, SBH Health System operates a Hospital Based Violence Interruption Program that combines the efforts of the emergency medicine staff with a trusted community based partner, Bronx Rises Against Gun Violence (BRAG), and runs boxing programs at their gym for girls and boys who have experienced gun violence.</p> <h2><span>Metropolitan Anchor Hospitals Need Support</span></h2> <p>Despite the financial challenges MAHs face, as well as the ongoing COVID-19 pandemic, SBH Health System is proud to have accomplished so much for its patients. But to be more impactful and adapt to a fast-changing health care landscape, MAHs like SBH Health System need adequate, sustained federal support. While SBH Health System has the potential to do even more for its community, it struggles with financial instability. Of the one million patients it serves every year, nearly 400,000 are uninsured, equating to more than $1.1 billion in uncompensated care. A majority of NYC H+H’s patients are insured by public payers, primarily Medicaid. Medicaid and Medicare visits represent roughly 85% of total annual discharges and approximately 70% of net patient revenue. The relatively small population of commercially insured patients typically visit the system’s trauma center, burn center, snakebite center, and behavioral health programs.</p> <p>More than 60% of NYC H+H’s patient population identifies as Black, Latino, or Asian Pacific Islander, and 30% of patients are Limited English Proficient (LEP). NYC H+H, like many MAHs across the country, faces significant financial challenges, low margins, and workforce shortages—all of which have been exacerbated by the COVID-19 pandemic.</p> <h2><span>NYC H+H's Unique Approach to Care</span></h2> <h3><span>Pioneering Programs to Expand Low-Income New Yorkers’ Access to Care</span></h3> <p>NYC Care is a health care access program that guarantees low-cost or no-cost services to New Yorkers who do not qualify for or cannot afford health insurance. Through NYC Care, patients receive a unique membership card to access health services through NYC Health + Hospitals, where they can select their own physician and receive affordable prescription medicines. More than 100,000 New Yorkers, 61% of whom live in the hardest-hit COVID-19 areas, now have access to health care through NYC Care.</p> <h3><span>Commitment to Behavioral Health</span></h3> <p>NYC H+H provides over 60% of the behavioral health services in New York City. While some systems in the city have closed their behavioral health units due to financial losses, NYC Health + Hospitals doubled down because of its mission to serve everyone. Its public hospitals and neighborhood health centers offer a wide variety of excellent mental health services to foster recovery, help patients rediscover their strengths and abilities, and overcome the stigma of mental illness.</p> <h3><span>Building the Infrastructure for NYC’s Ongoing Pandemic Response</span></h3> <p>The initial surge of COVID-19 cases in 2020 was a defining and terrifying moment for New York City. The NYC H+H system was at the “epicenter of the epicenter” of the pandemic’s first wave, treating thousands of patients while simultaneously leading outreach and care for the city. Within a few months of the first positive case in NYC, NYC H+H developed infrastructure and deployed staff to provide COVID-19 testing, contact tracing, isolation services, and later vaccines to all New Yorkers at no cost to them. In addition, NYC Health + Hospitals built three new community-based COVID Centers of Excellence to provide long-term COVID-19 treatment in areas hardest hit by COVID-19 in Brooklyn, Queens, and the Bronx. The pandemic exacerbated existing health inequities and aggravated ongoing health needs, making the work of NYC Health + Hospitals more important than ever.</p> <h2><span>Metropolitan Anchor Hospitals Need Support</span></h2> <p>Despite the financial challenges MAHs face, as well as the ongoing COVID-19 pandemic, NYC H+H has accomplished so much for its patients. But in order to be more impactful and adapt to a fast-changing health care landscape, MAHs like NYC Health & Hospitals need help. To empower metropolitan anchor hospitals like NYC H+H to continue serving structurally marginalized communities, Congress must create a federal designation for MAHs and provide adequate, sustained federal support. A rich tradition of leadership and service dates back to 1862, to the founding of Freedmen’s Hospital in Washington, D.C. The hospital was the first of its kind to aid in the medical treatment of formerly enslaved persons and African American servicemen returning from the Civil War. Housed in converted army barracks, Freedmen’s Hospital provided a refuge for patients receiving medical care due to the denial of care at other city hospitals. In the late 1860s, Freedmen’s formed a partnership with the Howard University College of Medicine to train African-American medical professionals. In 1909, Congress authorized the construction of a new hospital on the campus of Howard University. In 1967, Howard University was granted full authority over Freedmen’s Hospital. In 1975, the University opened a new hospital opened on Georgia Avenue NW, in the city’s historic Shaw neighborhood. The former Freedman’s building now houses the Howard University College of Nursing and College of Allied Health Sciences.</p><p>Over the course of its 160-year history of providing primary, secondary, and tertiary health care services, Howard University Hospital (HUH) has become one of the most comprehensive health care facilities in the Washington, D.C. metropolitan area that now has a designated Level 1 Trauma Center. HUH provides top-tier inpatient and outpatient health care services through its comprehensive set of [14] specialty care departments, including a Department of Emergency Medicine that sees approximately 50,000 patients annually. Further, HUH is home to a variety of centers specifically focused on wellness, cancer, cardiac, diabetes, and infectious disease management, among others.</p><p>A private, not-for-profit institution with nearly 1,700 employees, HUH is the nation's only teaching hospital located on the campus of a historically Black university. It is recognized as a national leader in training future doctors, nurses, and other allied health professionals. HUH offers medical students a superior learning environment, as well as opportunities to observe or participate in ground-breaking clinical research. Together, the medical school and the hospital serve as a training ground for the nation’s top black physicians focused on providing care to historically disenfranchised groups.</p><p>HUH’s mission is to lead in the advancement of health care and health equity, and its vision is to provide exemplary education, service, and research that promote patient-centered, high-quality, collaborative care and advocate for the elimination of health disparities. Respectively, Medicaid and Medicare revenue represents 83% of total net patient revenue and more than 76% of patient discharges. A small number of HUH patients have private, commercial insurance. Further, HUH provides more than $11.5 million in uncompensated care annually to support its promise of caring for patients regardless of their insurance status.</p><h2><span>HUH's Commitment to the Community</span></h2><p>Since the hospital’s inception, advancing health equity has been central to HUH’s mission. HUH sponsors numerous programs and initiatives to promote overall wellness and educate individuals on their health care needs and options to ultimately improve the overall health of the community:</p><h3><span>Faith-Based Partnerships</span></h3><p>By partnering with religious communities across the area, HUH is able to connect with individuals in a place they trust. These efforts are enabling HUH to improve individuals’ access to care before they are faced with a health care crisis, including much-needed mental and behavioral care.</p><h3><span>Emphasis on Prenatal Care</span></h3><p>Leadership at HUH has identified maternal health as a key area of focus. HUH’s diverse clinical faculty are dedicated to helping expectant mothers with lower family incomes overcome barriers in accessing maternal health care. Too often expectant mothers in the DC area struggle with prioritizing their prenatal care as they face challenges with managing employment, caring for their families, and struggling with lack of transportation. These barriers can result in greater risk for mothers and babies of birth in crisis; therefore, HUH has invested in a higher level NICU and has embraced a number of programs to improve access to prenatal care for expectant mothers, including birthing classes, nutritional education, and support wellness check adherence.</p><h3><span>Nutrition Education</span></h3><p>HUH recently participated with an online cooking show to educate individuals in the community on how to prepare nutritious and healthy meals on a budget. The goal is to help educate families to eat more healthfully while keeping grocery costs low. This program was launched to reach families and individuals who live in DC’s Ward 7 and Ward 8 who face sustained hardship.</p><h2><span>Metropolitan Anchor Hospitals Need Support</span></h2><p>HUH has a long and trusted history of providing consistent, quality care to individuals across the Washington, DC area, but financial resources are needed to make needed capital investments, including medical equipment upgrades to treatment for stroke victims and those with cardiac disease. Upgrades are needed to support a health information system for better patient data collection and the adoption of electronic health care solutions. In addition, more investment is needed to improve and maintain the hospital system’s existing infrastructure.</p><p>As faithful stewards of their mission, the team at HUH is dedicated to never turning away a single person in need. UAMS Health is on a mission to improve the health and well-being of Little Rock residents and patients across the entire state through its <a href="https://web.uams.edu/wp-content/uploads/2022/04/Fast-Facts-04_2022-1.pdf" target="_blank" title="UAMS Fast Facts">statewide health system</a>, which includes UAMS Health; a statewide network of campuses providing primary and specialty care; and, expertise in population health, digital health, health informatics, and research.</p> <p>UAMS maintains a 535-bed hospital on its main campus with the only adult Level 1 Trauma Center in Arkansas. UAMS’ centrality also makes it a region-wide magnet for high-quality care—some of which is offered nowhere else in Arkansas. The system operates the state’s only adult sickle cell clinic, adult spina bifida clinic, adult cystic fibrosis clinic, ALS treatment center, liver transplant program, kidney transplant program, high risk maternal-fetal medicine program, and cord blood bank.</p> <p>UAMS offers primary care at three off-campus locations in Little Rock and nearby Maumelle. Its eight family medical centers at regional campuses across the state recorded approximately 120,000 visits in 2020. Located between one of the wealthiest communities and one of the lowest-income ZIP codes in the state, UAMS treats patients with varying and complex care needs.</p> <p>Approximately, 20% of patients treated at UAMS are Medicaid beneficiaries and 30% are covered by Medicare. Medicaid and Medicare patients make up more than 52% of total visits and nearly 50% of total discharges, yet make up only 44.4% of net patient revenue. More than 40% of patients have commercial health insurance. Only about 2.5% of patients were uninsured, down from 19% prior to the implementation of the Affordable Care Act. UAMS provides an average of more than $81 million in charity and uncompensated care annually.</p> <p>UAMS is proud to serve a diverse group of patients. Approximately 15% of admissions are patients transferring from other hospitals seeking high-quality, specialty care.</p> <h2><span>An Institution Anchored to Its Community</span></h2> <h3><span>Meeting the Unique Needs of All Local Communities</span></h3> <p>The main campus of UAMS services a variety of different communities with various needs, all of which UAMS is able to meet, demonstrating its role as a true anchor to the community. To the north lie several affluent neighborhoods whose residents receive primary care from other providers but depend on UAMS Health for specialty and emergency services. South of the campus, across the highway, are several neighborhoods that have no grocery stores, pharmacies, or banks. UAMS knows that these communities have different needs and face different social determinants of health (SDOH), which is why it collects SDOH information from every patient at the point of care. Understanding that a variety of factors such as household income, exposure to violence, and food security impact patient health helps UAMS’ medical providers address health inequities.</p> <p>Recognizing the need to be a trusted member of the community, UAMS partners with several community organizations to bring health care and social services to traditionally marginalized areas. For example, a partnership to establish a healthy grocery store is underway. This commitment to equity also extends to supporting the UAMS workforce, especially those who come from low-income communities. The health system recently committed to paying a living wage for all employees, and through a partnership with local banks, builders, and the City of Little Rock, UAMS is working to rehabilitate old homes and provide low-interest loans to employees to purchase them.</p> <h3><span>Training Future Health Care Professionals – and Educating About Health Equity</span></h3> <p>UAMS plays an enormous role in educating Arkansas’ health care workforce, producing nearly half of the state’s practicing physicians. This commitment extends far beyond the Little Rock area. In rural areas of the state, which often face a shortage of doctors, UAMS trains an even higher percentage of physicians—nearly 60%.</p> <p>A significant part of the curriculum includes mandatory training education around health equity and social determinants of health. UAMS staff are also required to attend educational seminars on intrinsic bias and health disparities, and they have the opportunity to volunteer with the health system’s clinics that provide free care in low-income communities.</p> <h2><span>Metropolitan Anchor Hospitals Need Greater Support</span></h2> <p>Like many other metropolitan anchor hospitals, UAMS faces serious financial pressures. Without stable financial support, the specialized programs and extensive community outreach activities that benefit the Little Rock community will be at risk. With a MAH designation and adequate funding, systems like UAMS would have the opportunity to ensure financial stability. Established in 1877, Valleywise Health provides quality care to the people of Maricopa County in south-central Arizona and beyond.</p><p>Today, Valleywise Health is well known for operating regional health centers across rapidly growing Maricopa County, providing “exceptional care, without exception, to every patient, every time.” The system provides approximately 550,000 patient visits annually at a 325-bed medical center, the nation’s second largest burn center, a Level 1 adult trauma center, multiple behavioral health centers, Federally Qualified Health Centers, an HIV specialty clinic, and a dedicated refugee center.</p><p>Valleywise Health is also home to the Phoenix area’s only public teaching health system. One of the oldest and largest graduate medical education programs in Arizona, it became an approved teaching hospital in 1952. Through the Creighton University - Arizona Health Education Alliance, Valleywise Health trains the area’s next generation of physicians, nurses, and allied health professionals.</p><blockquote><h3><span><em>“At the end of the day, we are the poster child of a safety net hospital.”</em></span></h3><p><span><strong>Steve Purves</strong></span><br>President & CEO of Valleywise Health</p></blockquote><h2><span>Patient–Payer Mix</span></h2><p>Valleywise Health provides access to health care services to a disproportionately higher number of low-income and uninsured people in Maricopa County. Valleywise treats a high number of Medicaid and Medicare beneficiaries: both programs account for 85.7% of all visits, 65% of all discharges, and 41.4% of net patient revenue. Each year, Valleywise provides more than $44 million in uncompensated care.</p><p>Valleywise Health also delivers care to a diverse patient population. An estimated 57% of the patient population in ambulatory care settings are Latinx and nearly 10% are Black. More than 30% of patients are non-English speaking. Among its pediatric patients, 74% are Latinx, and 96% receive benefits through Arizona's Medicaid agency (Arizona Health Care Cost Containment System - AHCCCS), the Valleywise Health Financial Assistance Program, or are self-pay.</p><h2><span>Valleywise Health's Unique Approach to Care</span></h2><p>One aspect of Valleywise Health that makes it unique is that it is directly funded by the residents of the county it serves. The Maricopa County Special Health Care District Board of Directors – representing each of the county’s five districts – is the governing body for the district and Valleywise Health. In 2014, the county electorate voted to fund construction of the system’s new medical center, which helped transform this vital safety-net and public teaching hospital.</p><p>With financial support from the local community, Valleywise Health is able to offer the residents of Maricopa County access to world-class health care that addresses the medical, behavioral, and social needs of the community.</p><h3><span>Integrated Behavioral Health (IBH) Services</span></h3><p>Valleywise Health provides access to the full spectrum of behavioral health services and timely, compassionate, and evidence-based treatment.</p><h3><span>Family Resource Centers</span></h3><p>Valleywise Health offers educational programs such as parenting, literacy, computer, and cooking classes, as well as a number of supports and tools that promote healthful living.</p><h3><span>Women’s Refugee Services</span></h3><p>Multilingual, multicultural members of the refugee community serve as Cultural Health Navigators to educate and empower patients to access health care services. Additionally, the clinic hosts classes to help expectant mothers prepare for childbirth and newborn care. Through this program, Valleywise serves more than 3,000 women and children annually from 49 countries speaking 41 languages.</p><h2><span>Metropolitan Anchor Hospitals Need Greater Support</span></h2><p>Despite having made strides in improving clinical and community programs, operating efficiency, and expense management, Valleywise Health still faces challenges in meeting the needs of Maricopa County’s largely low-income and uninsured citizens. While some challenges of the pandemic have subsided, Valleywise Health continues to face financial insecurity, with no resources currently available in their safety net fund. Additional safety-net funding is needed to support greater investment in the community, particularly related to maintaining an adequate health care workforce, which has been Valleywise Health’s greatest challenge during the COVID-19 pandemic.</p><p>Valleywise Health, while unique in many ways, faces the same financial pressures as MAHs across the country. Without stable support, specialized programs designed to serve Valleywise Health’s traditionally marginalized populations will be put at risk.
