Castell also will provide digital tools for virtual care, patient experience and social determinants of health, as well as provide access to Intermountain’s latest innovation initiatives.</p> <p>In its first year, the Reimagined Primary Care model has led to:</p> <ul> <li>A 60% cut in Medicare Advantage admissions.</li> <li>25% fewer commercial insurance admissions.</li> <li>A 20% reduction in per member per month costs.</li> <li>Improved patient ratings and physician satisfaction.</li> </ul> <h2>What’s Going On</h2> <p>Intermountain is leveraging its deep knowledge in improving quality, reducing costs and effectively managing downside risk in value-based care programs to improve access. Its leaders say they are shifting to a more community-based mindset and that Castell will provide benefits to adjacent markets in Utah and Nevada. This could help participating providers in these areas to improve efficiency and outcomes and cut costs as they participate in value-based care models.</p> <p>The launch of Castell also is part of a broader trend among some health systems to diversify their business lines outside of reimbursable medical services. These innovators are creating companies in areas like technology, automation, group purchasing and supply chain management. And with the field likely being several years or more away from assuming full risk in value-based payment models, others are stepping up with services to help ease the transition.</p> <p><a href="https://www.fiercehealthcare.com/hospitals-health-systems/providence-st-joseph-health-launches-population-health-company" target="_blank">Providence St. Joseph Health</a>, for instance, earlier this year launched <a href="https://www.ayin.com/" target="_blank">Ayin Health Solutions</a>, a population health company that offers expertise to help payers, providers, employers and government entities reduce costs and improve care in the transition from inpatient to ambulatory care. <a href="https://www.evolenthealth.com/" target="_blank">Evolent Health</a>, an independent company formed by <a href="https://www.upmc.com/" target="_blank">UPMC</a> and <a href="https://www.advisory.com/" target="_blank">The Advisory Board</a> in 2011 to provide an integrated population and health management platform, is a $1 billion company today focusing on helping providers and health plans succeed in value-based care. However, it could be some time before the bulk of the field assumes full risk in value-based payment models, as a <a href="https://inspire.changehealthcare.com/2019-Industry-Pulse-Whitepaper" target="_blank">HealthCare Executive Group and Change Healthcare survey</a> of health care executives found earlier this year. But that reality may not materialize as quickly as some envision, based on results of the <a href="https://inspire.changehealthcare.com/2019-Industry-Pulse-Whitepaper" target="_blank">9th Annual Industry Pulse Survey</a> recently released by <a href="https://www.changehealthcare.com/" target="_blank">Change Healthcare</a> and the <a href="https://hceg.org/" target="_blank">HealthCare Executive Group</a>.</p> <p>The survey found that nearly two-thirds of responding hospital leaders, third-party administrators, health plan executives, academics and other stakeholders believe that VBC won’t be dominant for another three to five years or more. And 6 percent believe that it will never happen.</p> <p>What’s hindering progress? Respondents cited obstacles like limitations in data sharing; no agreement on outcome measures; and a lack of incentives for payer and providers to work together. All of this has left health care leaders feeling unable to tackle the challenges of implementing and managing these contracts, the report concludes.</p> <p><img alt="Chart: When to you expect the majority of value-based relationships within the health care market to contain both upside and downside risk?" data-entity-type="file" data-entity-uuid="cde4e18f-aa53-4743-8917-4541127cc0b2" src="/sites/default/files/inline-images/ms_052119_item1_chart-03_v2.jpg" width="333" height="333" class="align-right">Making rapid change on such a large scale also may be an impediment, with the report noting that VBC will require major changes in business models — beyond simply operationalizing the steps from concept to contract to claims processing. A new AHA Market Insights report <a href="/center/emerging-issues/market-insights/evolving-care-models">Evolving Models of Care</a> delves much deeper into the successes and challenges providers face in aligning care delivery models with emerging payment models.</p> <p>The Pulse Survey also explored ways that payers can more effectively support providers in facilitating high-value care. Standardizing quality and outcome measures was the most common suggestion, cited by 28 percent of the respondents, followed by co-developing payer-provider risk-management programs (22.6 percent), sharing performance data (14 percent) and co-developing bundled payments around episodes of care. ms_052119_item1_chart-03_v2.jpg</p> <p>Pulse Survey respondents plan to take actions within 12 months to address nonmedical barriers to care. These are the top five:</p> <ul> <li><strong>Care coordination:</strong> Slightly less than two-thirds of respondents will work to better coordinate care.</li> <li><strong>Transportation:</strong> Nearly 40 percent of organizations plan to make it easier for patients to get to their appointments. A growing number of providers nationally are working with Lyft and Uber to transport patients to doctor visits and hospitals, helping to avoid the costs of delayed or missed care.</li> <li><strong>Food insecurity/access:</strong> More than one in four respondents will address this social determinant of health. The company’s customers, which include insurers and self-insured employers, pay only for the patients whose outcomes are improved, <a href="https://www.forbes.com/sites/elliekincaid/2019/04/01/better-diabetes-care-healthcare-startups-say-theyll-get-paid-when-patients-get-better/?ss=healthcare#2b3a051020c2" target="_blank">according to Forbes</a>.</p> <p><a href="https://www.crickethealth.com/" target="_blank">Cricket Health</a>, which has raised more than $25 million from investors including Cigna and First Round Capital, helps patients with chronic kidney disease to put off the need for expensive dialysis. Cricket staff (a mix of social workers and clinicians) review the medical records of insurers’ patients to spot those at risk for developing chronic kidney disease. They also educate patients about how to get on a kidney transplant list and the benefits of in-home dialysis vs. a clinic setting, and help them get the treatment they need to stay healthy. The lower the cost of medical care needed, the more money Cricket receives.</p> <p><a href="https://onduo.com/" target="_blank">Onduo</a>, a joint venture in diabetes care from the drugmaker Sanofi and Verily, the life sciences company from Google’s parent company Alphabet, is taking its pay-for-performance commitment a step further. It requires customers of its tech-based diabetes management solution to pay based on outcomes. Earlier this year, Walgreens began offering its employees with type 2 diabetes Onduo’s suite of connected glucometers, software that tracks trends in the numbers and specialists available via telemedicine.
