CDC to fund 40 collaborations for addressing intersection between social determinants of health, chronic disease
The Centers for Disease Control and Prevention recently issued a notice of funding opportunities for collaborations between multisectoral partners to develop implementation-ready social determinants of health accelerator plans. CDC said it will fund approximately 40 applicants. The program is intended to reduce disparities in health outcomes related to chronic disease within a state, local, territorial, and tribal jurisdiction, community or catchment area. This is the second year that the AHA-supported program has been funded. The deadline for applications is July 8 by 11:59 p.m. ET.
