Three programs that expand the reach of palliative and end-of-life care will receive 2018 Circle of Life Awards July 27 at the AHA Leadership Summit in San Diego. They are: Hospice of the Valley, Arizona Palliative Home Care Program, Phoenix; Penn Wissahickon Hospice and Caring Way, Penn Medicine, Philadelphia; and Palliative Care, Western Connecticut Health Network, Danbury, CT. In addition, three programs will receive citations of honor: Rainbow Kids, Intermountain Primary Children鈥檚 Hospital, Salt Lake City; Department of Supportive Care Medicine, Moffitt Cancer Center, Tampa, FL; and Hand in Hand Palliative Care, Children鈥檚 Hospital & Medical Center, Omaha, NE. 鈥淭his year鈥檚 honorees demonstrate that the needs of patients and their families can be met at home and in other community settings to ensure that the best possible care is provided across the care spectrum,鈥 said AHA President and CEO Rick Pollack. 鈥淭hese innovative programs address critical needs that help patients best manage their health care needs.鈥 For more on the Circle of Life Award and 2018 honorees, see the AHA news release.
