The AHA’s Health Research & Educational Trust recently that it will partner with the University of Kentucky Center for Health Services Research to conduct a study to determine transitional care services that improve health outcomes and are of high value to patients and their caregivers. The study, called  – Achieving Patient-Centered Care and Optimized Health In Care Transitions by Evaluating the Value of Evidence – is funded by the Patient-Centered Outcomes Research Institute. “We are thrilled to be working with the University of Kentucky on the Project ACHIEVE Award that will build on our national quality improvement work,” said Maulik Joshi, HRET president and AHA associate executive vice president. “This work will advance patient care and improve the overall care process.” The three-year project will involve the design and development of best practice recommendations and implementation guidelines for national patient-centered care transition programs, and emphasize the importance of patient transitions from hospital to home as well as the evaluation of nursing facilities and community care transitions.
