Community Health Improvement: Stories from the Field
We鈥檙e looking for stories about community health improvement from hospitals, health systems and community partners working to improve the health and well-being of individuals and communities and advance health equity. Stories can be submitted in a variety of formats, including narratives, video and images. Submitted content may be selected and shared via AHA newsletters, websites and social media platforms.
Do not send any protected health information, such as photographic images of patients or other identifiers, without first obtaining a HIPAA Authorization from the patient that specifies the information may be shared with the AHA and publicly.
