Readiness for an Award

How do you know if your hospital or health system is ready to apply?

The program described in the application must be operational for at least 2 years.
The program must have demonstrated positive impact in the community, and documentation of or data on the program’s effectiveness is available.
The program is sustainable and can be replicated in other communities.
The program improves equity in your community.
